QUA-08 FMEA and Reverse FMEA

One of rare  “standard” training modules in ABP Management’s offer. But, there’s a reason behind that. Process FMEA is created by the process development team and even if there should be a cooperation between the R&D and production plant, most often the production plant creates the PFMEA alone. We have no doubt that employees know the rules for FMEA preparation. Nevertheless we experience situations when this powerful tool is underestimated, RPN adjustment to reach some audit objectives or just standard FMEA copied for the new project. Training module FMEA and Reverse FMEA is based on common AIAG-VDA recommendations (new from 2018) and includes specific requirements of some customers on this tool. In recent years also Reverse FMEA is more and more used and several customers require it from suppliers (e.g. PSA Groupe, Renault, ...). Its meaning for quality assurance and application is a part of this training module.

Even the best production process will not produce a compliant part if the incoming material is non-conform. In the world of today’s automotive industry, and despite theories and internal procedures, the main decisive factor remains the price of a purchased component. Typical “problematic” suppliers are internal sub-suppliers, sub-suppliers imposed by the customer or Tier-2 suppliers for which the Tier-1 customer is not important from a turnover point of view. Again the question: “What can we do with that”?

The purchased part quality may be influenced by continuous work with sub-suppliers, clear expectations and rules and strategic partner development. Regular internal self-audit according to VDA 6.3 norm is one of the requirements of several German OEMs. The supplier must be trained on given norms, nevertheless, the practical use is not always efficient. Pareto principle – 80% of problems we experience in mass production have their root causes in pre-production phases. Managers do not have people to work on projects but there’s always still enough money to pay scrap & invoices and people to solve claims. And the “vicious cycle” repeats again.

Process FMEA is taken over from similar projects in the past, for SPC the best parts are taken or results “optimized” … Everything is perfect! But, and nobody knows why, the plant has a high internal scrap, claims from the customer or missing capacity… Those who accuse and those who are accused change their positions reciprocally.
